Best way to find the most important/landmark papers in medicine?

Anyone have any ideas? I've tried Journalwatch, but it's expensive and doesn't provide access to the original abstracts - it seems like it is just commentary. 
Someone sent me  http://signup.trendmd.com/ but it doesn't allow me access. Anyone used it?

Anyone have any ideas? I've tried Journalwatch, but it's expensive and doesn't provide access to the original abstracts - it seems like it is just commentary.
